Emergency Medicaid at Choices
Choices offers New York State Emergency Medicaid for abortion and for prenatal procedures to patients who live in Queens, Manhattan, Brooklyn, the Bronx, and Staten Island. Emergency Medicaid will also cover the follow-up visit after an abortion.
To find out if you qualify, please call Choices and ask for the Medicaid Department. You will be asked to come to the clinic with the proper documents, and you will find out immediately whether or not you qualify.
Please bring the following original documents with you:
- Valid photo ID (must be government issue and cannot be expired)
- Birth Certificate or Passport (if your Birth Certificate is in a foreign language, you must have a notarized translation to English)
- Social Security Card (if applicable)
- Proof of Address (must be under the patient's name and dated within 3 months. PO Box addresses are not accepted)
- Proof of Income OR, if you are a full time student under the age of 21, Proof of Enrollment
- Medicaid approval lasts for 45 days and does not cover the cost of prescriptions.
